In Peter's dream, the close up on his face.. He does have a scar on his upper lip, opposite of his drooping lower lip (I think the scar is on the upper right lip).
So is this THE scar? Or does Milo or his character always have this scar?
Also if this is THE scar... Why the heck would Hiro say he didn't recognize Peter w/o it if it is so small?

Nah, I think he always had that little scar. The one that Hiro was refering to must have been a big one. It must be big enough to alter his apperance quite noticibly.
Hiro says " You look different without the scar". He didn't say I didin't recognize you without the scar

im guessin the scar is gonna be a huge one across his forehead... He'll get it before he meets Hiro, so it should be soon..

Alternatively, Hiro has seen Peter for so long with the scar that he'd forgotten what Peter looked like before he had it.

Ooo, good point! This raises a lot of questions. Does something happen to Claire anyway? Have Peter's powers changed so that he can't mimic her? Do Peter and Claire lose contact in the future due to a falling out? Do they lose contact due a future war that has our heroes operating in resistance cells, split up all over the world? Am I running wild with speculation and over analyzing this one point? Most likely...

Maybe Peter get the scar even with Claire around (since he is just mimicking her powers, maybe he can't "fix" himself properly) or he gets the scar while training with his new mentor (the invisible man).

But if he saved Claire, considering he changed the future as scullymulder1234 said, don't you think he may not get the scar too? Or am I becoming crazy with these time travel theories?
And speaking about the invisible man, I can't wait to see his interaction with Peter! I'm sure it's going to be awesome!

I agree with chrissstopher's theory about why Hiro didn't recognize Peter without the scar.
